New Practices
Every healthcare organization that got its first NPI in the last six months, by state and practice type, with the owner behind each one and the operators opening several sites at once.
Open New Practices -- in the sidebar under Targeting.
New Practices answers "who just opened, and who is behind them?" A practice that registered a few weeks ago is still choosing its EHR, billing service, suppliers and referral partners. The page lists every new practice in your scope, finds the person who filed each one, and shows which operators are opening several sites at once.
Where the data comes from
The source is the CMS NPPES NPI registry. A practice counts as new when its organization NPI was first issued in the window shown on the page:
- The window is the six most recent months in the registry. The date chip next to the pickers shows the window (for example "May-Oct 2026") and the newest issue date in the registry ("registry through ...").
- Only organization NPIs count. Organization subparts (a department or site filed under a parent organization) are left out. Individual clinicians are not counted.
- Only the eleven practice types below count. All practice types means all eleven together, not every organization in the registry.
Picking a scope
Two pickers sit under the header card:
- State -- All states or one state.
- Practice type -- All practice types or one of:
| Practice type | What it covers |
|---|---|
| Behavioral health | Behavioral health and social service providers |
| Clinics and centers | Ambulatory health care facilities |
| Physician practices | Allopathic and osteopathic physician groups |
| NP and PA practices | Physician assistant and advanced practice nursing groups |
| Dental | Dental providers |
| PT, OT, speech and hearing | Physical therapists, occupational therapists, speech-language pathologists and audiologists |
| Chiropractic | Chiropractic providers |
| Eye and vision | Eye and vision services providers |
| Podiatry | Podiatric medicine and surgery providers |
| Nutrition | Dietary and nutritional service providers |
| Home health and care agencies | Agencies |
State and practice type live in the URL, so a scope is shareable as a link. Changing either one returns you to the Newest tab.
The header
The title shows the state (linked to its Territory Brief) or United States.
Key signals
Up to five plain-language lines, built from the data on the page:
- How many practices opened in the full months of the window, and the change on the same months a year earlier (up, down, or flat when the change is under 2%)
- Fastest riser -- the specialty growing fastest on last year (or the practice type, when no specialty qualifies)
- The biggest multi-site operator: how many sites it opened, across how many cities, and how many new practices belong to multi-site operators
- The city with the most openings, and the city growing fastest year on year
- The share of all organizations in scope that got their NPI in the window
The operator, city and specialty lines need a state or a practice type picked (see Operators, Buildings, Specialties and Cities).
Opened per month
A small bar chart of each month in the window against the same month a year earlier. When the newest month is not complete in the registry, its bar is striped ("so far") and it is left out of every year-on-year figure.
Headline counts
| Count | What it shows |
|---|---|
| New practices | Practices with a first NPI in the window |
| vs last year | Change against the same months a year earlier, full months only |
| New share of market | New practices as a share of all organizations of the chosen types in scope |
| Per 100k residents | New practices in the full months per 100,000 residents of the state (or the US) |

Newest
"Who just opened": the 50 newest practices in scope, newest first, 20 at a time (Show N more loads the next 20). Each row shows:
- The practice name, linking to its record page
- Its practice type (when all types are shown) and specialty. The specialty links to the Providers table filtered to organizations with that specialty in the state.
- The street address, which opens the location on a map
- The practice phone and the date the NPI was issued
- An Owner button (see Owner lookup)
With a scope picked, a Biggest opener in ... card sits beside the list: the largest multi-site operator, its phone, how many new sites it opened, its five newest sites, and an All operators button that opens the Operators tab.
Filter by owner narrows the list to Multi-org owners or First practice once owners are found. The line next to it counts how many owners have been found.
Operators
"Who is opening several at once": new practices grouped by the phone number filed with each NPI, since one contract can cover every site. Only phones shared by two or more new practices count. Three totals sit at the top: practices that belong to an operator opening several sites, practices that share a phone with differently named practices, and single, independent openings.
| Kind | Meaning |
|---|---|
| Multi-site launch | One organization name registered at several sites |
| Brand family | Different entities under one brand name |
| Shared phone | Unrelated names behind one phone: a group running several brands, or a billing or credentialing firm filing for its clients |
Each operator shows its site count, cities, newest issue date, up to eight sites and the phone. The name opens every practice filed with that phone in the Providers table. The 30 largest groups are shown.
Buildings
"Buildings filling up": street addresses where two or more new practices registered in the window -- one visit, several new accounts. Suites are grouped into one building. Each building lists its new tenants with specialty and issue date, and a Map link.
An address with 20 or more new practices is tagged Shared address: likely a virtual office; such an address is usually a virtual office or registered-agent address, not a clinic. The 30 busiest buildings are shown.
Specialties and Cities
Both tabs have the same two halves:
- Opening most -- the top 12 by new practices in the window, each with its change on last year.
- Moving most vs last year -- the ten with the largest change in openings, full months against the same months a year earlier. Specialties or cities with fewer than 20 openings last year are left out.
Specialty is the primary specialty filed with each new practice. Names link to the Providers table filtered to organizations with that specialty or in that city.
States
"Where practices are opening": every state for the chosen practice type, full months only. Sort by Most new, Per 100k residents or Fastest growing. Fastest growing leaves out states with fewer than 30 openings last year; per 100k uses Census 2025 state population. Click a state name to scope the whole page to it, or Territory brief to open its brief.
By type
Every practice type with its new practices in the window and its change on last year. Click a type to scope the whole page to it. On wide screens each row also has Open in Providers and Save as lead list.
Trend
The month chart at full size, with each month's count and its change on the same month a year earlier.
Operators, Buildings, Specialties and Cities
These four tabs read every new practice in scope, plus the same months a year earlier. They need a state or a practice type picked; with neither, the tab asks you to pick one. When a scope is still too large to read at once, the tab says so -- pick a state or a practice type to narrow it.
Owner lookup
Owner opens the person who filed the practice's NPI (the authorized official in the NPI registry):
- Owner of record -- name, title and direct line, with a Call owner button.
- Practicing clinician -- when exactly one clinician NPI in the state carries the same name, the owner's own clinician record, specialty and how long they have held an NPI. When several clinicians share the name, the page says the owner cannot be pinned down. When none does, the owner is likely a business owner or investor rather than a practicing clinician.
- Controls N organizations in ... -- other organizations in the state that list the same official, newest first, with city and issue date. "same phone" marks the ones that share the official's phone. When there are none, the panel reads First organization in ....
Matching is by the official's name, so a common name can pull in someone else's organizations; "same phone" is the stronger signal.
Once found, the owner also appears on the row with a tag: +N other orgs, First practice, or how many organizations they opened in the last 12 months.
Find owners for N shown looks up every row on screen that has no owner yet. It stops early if the registry is busy or your credits run out.
Owner lookup needs an active plan, and each lookup uses one credit. If the registry has no authorized official for a practice, or is busy, the panel says so; try again in a few minutes.
Watch a scope on Radar
With a state or a practice type picked, Watch next to the pickers adds the scope (for example "Dental in Texas") to Radar under New practices. Radar counts the practices that appear in the NPI registry after your last look.
- The New Practices item in the sidebar shows a badge with the number of new practices across your watched scopes since your last visit. A scope that sits inside another watched scope (Dental in Texas inside all types in Texas) is counted once.
- Changes also reach the Radar bell and the weekly digest, like any other Radar watch.
- Opening a watched scope on New Practices marks it as seen. A chip next to the button shows "+N new since your last visit".
- On Radar, each watch shows New since last look and how many opened in the newest month in the registry.
Click Watching to remove the watch. The button appears only when a state or a practice type is picked.
Export and lead lists
- Export (Newest tab) downloads a CSV of the listed practices: NPI, practice, practice type, specialty, address, city, state, ZIP, practice phone, NPI issued, and -- for rows whose owner you have found -- owner, owner title, owner phone, owner organizations in state, owner orgs opened in the last 12 months, owner clinician NPI and the year that NPI was issued. Run Find owners first to fill the owner columns. Exporting needs an active subscription and counts as one export against your plan's allowance.
- Save as lead list appears on the Newest tab once a practice type is picked, and on each row of the By type tab. It saves the practices of that type in scope, newest first, as a new lead list of up to 500 leads. When the window spans two calendar years, there is one link and one save button per year.
- Open in Providers opens the same practices in the Providers table.
Organization record pages
On an organization's record page, the Organization tab shows when its NPI was issued. An organization whose NPI was issued within the last six months is marked New practice, and a link opens New Practices for its state and practice type.
Limitations
- Counts follow NPI issue dates. A practice that existed before under another NPI, or opened without a new organization NPI, does not appear.
- Only the eleven practice types above are counted.
- Operators are grouped by phone, so a billing or credentialing firm that files for many clients shows up as a Shared phone group, not an operator.
- Owner matching is by name; check "same phone" before treating a long organization list as one owner.
- Year-on-year figures use full months only.
- Opening the page uses one credit; each owner lookup uses one more. See Usage and Limits.
