Route of Administration
The route of administration is the path by which a drug enters the body, such as oral, intravenous, subcutaneous, intramuscular, topical or inhaled. It shapes how fast and how completely the drug reaches its site of action.

What is a route of administration?
Enteral routes use the digestive tract, such as oral, sublingual and rectal. Parenteral routes, in everyday use, mean injections that bypass it, such as intravenous (IV), intramuscular (IM) and subcutaneous (SC). Topical, transdermal, inhaled, nasal and ophthalmic routes deliver drugs through skin or mucous membranes, either locally or into the bloodstream.
Route affects bioavailability, onset and who gives the drug. An oral tablet is usually taken by the patient at home, while an IV infusion requires a clinic, infusion center or hospital.
An active ingredient approved for more than one route is typically marketed as separate products, sometimes under separate applications and trade names.
How route shapes payment and access
In U.S. coverage, route largely decides which benefit pays for a drug:
- Pharmacy benefit: self-administered products, such as oral drugs, most inhalers and many self-injected pens, are usually dispensed by pharmacies and covered under Medicare Part D or commercial pharmacy benefits.
- Medical benefit: drugs given by a clinician, such as IV infusions, are usually billed by the provider with HCPCS codes and covered under Medicare Part B or commercial medical benefits.
- Self-administered drug exclusion: Medicare Administrative Contractors publish lists of drugs they consider usually self-administered, which Part B generally does not pay for when given incident to a physician service.
- Site of care: payers may steer infusions from hospital outpatient departments to lower-cost home or infusion center settings.
Why route of administration matters
Route is a practical filter for competition, targeting and data work:
- Equivalence: generics and biosimilars must share the route of their reference product, so a new route can create a distinct product with its own competitive position.
- Lifecycle strategy: moving a biologic from IV infusion to subcutaneous injection can change where it is given and which benefit pays for it.
- Targeting: route predicts where a drug is used, so IV products point to infusion centers and hospital outpatient departments while oral products point to prescribers and retail pharmacies.
- Data joins: route is a separate field in drug labels, the NDC Directory and the Orange Book, and filtering on it separates products that share an ingredient name.
